         REGISTRATION DATES FOR 2010      AUGUST 2nd & AUGUST 17th (6-9pm)AT CENTENNIAL MIDDLE SCHOOL

 To speed registration process, please carefully read packet and complete all forms before arriving.

 We will not have internet access at CMS, so USA Hockey registration must be done prior to registration night.

Players WILL NOT be allowed to register if there are outstanding balances unless payment arrangements have been made with the treasurer . If no attempt is made to pay off the balance due - we will be turning  outstanding balances over to a collection agency. It is your responsibility to make arrangements with the treasurer to make sure your player is in good standing.

The following items 1 - 5 are REQUIRED to register for hockey:

1. USA Hockey On-Line Registration - MAKE SURE YOU ENTER  2010-11 for the season!!!!!!

You must register your child with USA Hockey and pay the $30 USA Hockey fee and $10 MN Hockey fee on-line prior to in-person registration.  Print off and bring with you, your bar-coded confirmation page.  Any child 6 or under (born January 1st, 2004 or later) - DOES NOT PAY EITHER FEE, but must still register with USA Hockey prior to in-person registration. CREDIT CARD IS NEEDED.  IF NONE AVAILABLE, YOU CAN PURCHASE A VISA GIFT CARD AT MANY STORES TO USE ONE TIME ONLINE.

Your Confirmation receipt also includes the USA Hockey Waiver of Liability (which you agreed to while registering on-line)

USA Hockey Registration (online) must be done every year.  The membership is only valid for one season so be sure to choose 2010-11.

5..  COPY of  player's govt issued birth certificate (if not on file with CYHA)

(If player was on Mini Mite team in 09/10 SEASON a birth certificate is not on file and will need to be provided)

To determine the correct playing level (initiation, squirt, 10U girls etc), click on the Age Chart below.  Please NOTE - players with July or later birthdays are eligible to follow their grade and register to play at the next level.  This will be verified.
